5 reasons your mock recall doesn't test your product recall program. Food and consumer goods companies conduct (or should conduct) regular mock recalls to test their product recall procedure or their product recall plan. Product recalls can pose a significant reputation risk to a company, and quickly escalate into a crisis so getting a product recall right is critical. The recall is an effort to limit ruination of the corporate image and limit liability for corporate negligence, which can cause significant legal costs. It can be difficult, if not impossible, to determine how costly can be releasing to the consumer a product that could endanger someone's life and the economic loss resulting from unwanted publicity. Recalls are costly. Costs include having to handle the recalled product, replacing it and possibly being held financially responsible for the consequences of the recalled product. A country's consumer protection laws will have specific requirements in regard to product recalls. Such regulations may include how much of the cost the maker will have to bear, situations in which a recall is compulsory (usually because the risk is big enough), or penalties for failure to recall. The firm may also initiate a recall voluntarily, perhaps subject to the same regulations as if the recall were compulsory.

Deficiencies in complaint and Recall Handling cause warning letter. Due to deficiencies in the investigation of the scope and root causes of reported complaints, the U.S. Food and Drug Administration (FDA) has issued a warning letter to an Indian drug manufacturer. The FDA inspection already took place in December of 2016. Because of the non-satisfactory reply in regards to the inspection findings by the manufacturer, the FDA issued the warning letter in August 2017. The FDA inspectors noticed that investigations into process deviations and out-of-specification (OOS) laboratory results were insufficient, and did not include scientifically-supported conclusions. According to FDA the manufacturer´s response to this observation was inadequate because, amongst others, it failed to fully investigate the scope and root causes of a reported complaint. The FDA now expects the manufacturer to retrospectively review all complaint, manufacturing, and laboratory investigations associated with each product that is produced for the U.S. market. Furthermore, a plan to ensure that all future investigations are thorough, scientifically sound, and result in appropriate and effective CAPA (corrective actions and preventive actions) should be provided. Another major deficiency was that the firm failed to follow the company´s own written procedures regarding the recall of failing products (for example: after receiving a complaint sample the defect has been confirmed, but a product recall as directed by company´s own procedures regarding recalls of defective products has not been initiated in time). Therefore, the FDA demands to provide a list and summary explanation for all other instances in which product(s) distributed within the last five years failed to meet established specifications, but for which actions prescribed in the firm´s Quality System Manual and recall procedure have not been taken. Additionally, the planned CAPA for each such instance should be provided including an explanation of CAPAs for ensuring to follow own procedures regarding product quality and recalls. In addition the FDA observed deficiencies in cleaning operations. The authority demands sampling procedures and analytical methods used to test residues found during the inspection, including validation protocols and validation reports. Furthermore, a plan is required to ensure that personnel responsible for cleaning, verifying equipment cleanliness, and maintaining equipment are appropriately trained and capable of performing their assigned duties. An overall assessment of the adequacy of your cleaning program for all equipment, with special emphasis on difficult-to-clean parts should be provided. Another deficiency was found regarding Process Controls. The FDA observed that the firm "does not have an adequate ongoing program for monitoring process control to ensure stable manufacturing operations and consistent drug quality". FDA’s guidance document, Process Validation: General Principles and Practices, for general principles and approaches that FDA considers appropriate elements of process validation, should be followed.

Low Quality Nugs Who knows what kind of meats they mash together to make those chicken nuggets. One ingredient no one wants is plastic, but that’s exactly what some people got in 2016 with certain Tyson Food products. The company recalled around 132 thousand pounds of fully cooked chicken nugget products. The Food Safety Inspection Service said that Tyson inspects it's chicken with a metal detector but that it couldn’t find plastic. 6. Tainted Cereal There’s just nothing like a bowl of your favorite cereal in the morning or anytime of day. Imagine opening a fresh box of say Fruit Loops and discovering an overwhelming aroma, not of sugar and artificial fruits, but instead something, stale, metallic and soap like. That would suck, but it's exactly what happened to a bunch of people back in 2010. Certain boxes of Kellogg's Fruit Loops, Apple Jacks, Honey Smacks and Corn Pops had box liners with that unusual smell and taste. Kellogg said that it was from a substance in the resin used to make the plastic liners. People got sick and the Kellogg company was forced to recall around 28 million boxes. 5. Toyota’s Black Eye Toyota agreed in early 2014 to pay a ridiculous 1.2 billion dollars to avoid being prosecuted for covering up one minor detail about some of their cars, they experienced what was called unintended acceleration. A slew of accidents caused by the malfunction, some of which were fatal finally started coming to light in late 2009. Court documents would reveal that Toyota knew of the problem months before the recall and didn’t tell anyone about it, clearing choosing financial gain over the safety of their customers. 4. Hairy Flour A faulty screen at a Morrison Milling Company subsidiary caused a huge recall. It’s crazy how one mistake can cause such a huge amount of waste but big companies should always air on the side of caution. Due to the possible presence of a hair like nylon fragment the company recalled thousands of 5 pound whole wheat flour packages that were distributed in the Southwestern United States. 3. Get Outta Here Kidde Kidde was forced to recall almost five million of their disposable fire extinguishers. Unfortunately it seems that someone on the manufacturing team forgot to check whether or not they actually worked. The company received nearly a dozen reports of the device failing to discharge, which sucks when you consider that those people all probably found out while trying to put out a fire. Apparently the problem was a faulty valve component. You know someone got in big trouble for that mistake. 2. Disease Causing Bacteria Found in Sanitizer That statement may seem contradictory and unbelievable but it's a descriptor of Clarcon products in 2009. The company issued a recall after it was discovered that several of their lotions and hand sanitizers contained high levels of disease causing bacteria, which is obviously the exact opposite of what sanitizer is supposed to be. Persil For years Persil was a popular brand of detergent under the Unilever umbrella. Then, in the early 90’s they unveiled Persil Power, which they touted as having new chemicals that increased cleaning performance. Unfortunately the new formula worked a little too well. After just a few washes, clothes would lose their color, and their structural integrity. Garments would rip and tear under the slightest stress. An embarrassed Unilever issued a product recall before abandoning the brand altogether, which is why most of you hadn’t heard of or knew what the hell Persil was until just now.

There are orange dots in a half moon shape on the face of the toaster in the left bottom corner. “Just like home” is printed in white on the front right bottom corner of the toaster. The plastic slice of toast, under pressure, can crack and break into small pieces posing a sharp edge or small part hazard to young children. Toys “R” Us received two incident reports with one report of the plastic bread shattering in Canada. No serious injuries resulted. About 174,000 baby dolls are posted for recall by the United States Consumer Product Safety who have said that the baby doll recall is "due to burn hazard." Apparently, a circuit board in the dolls' chest can reportedly overheat which causes the surface of the My Sweet Love and My Sweet Baby Cuddle Care baby dolls cause burns and blisters. The CPSC report notes that Walmart, which has exclusively distributed the dolls in the US since August 2012 until earlier this month, has received 12 incident reports with regards to the doll. http://www.crosstrainingenduro.com http://tractionerag.ca We have done a list of known issues and common problems for various dirt bikes, now it's time to look at the KTM known issues and KTM recalls. We also look at TPI problems and oil injection issues later. A common criticism of Husqvarna and KTM is they bring out some of their innovations too soon and customers become their guinea pigs, resulting in a lot of recalls and ongoing problems. So let's start with KTM product recalls and Husqvarna known issues. These Husqvarna and KTM recalls are the more serious problems because riders can get hurt. So the accountants and lawyers work out the potential cost of getting sued, versus the cost of actually fixing the mistakes. If it's cheaper to just fix the bike, they will issue a recall. On earlier Husqvarna and KTM models, you can see in certain models there were potential problems with fuel leaks, front forks seizing, front forks snapping, throttles locking open, or loss of braking. More recently in KTM & Husqvarna recalls there are still potential problems with fuel leaks and loss of braking, but also new issues with spokes, connecting rods and rear shocks on certain models. There are a lot of recalls on Husqvarna and KTM street bikes too but we'll leave that for someone else to cover. In 2017, KTM and Husky switched from the mostly trouble-free Keihin carburetor to the Mikuni. KTM Freeride air boxes were poorly designed. In wet weather the rear wheel would throw water up into the airbox and eventually drown the engine. In 2017 KTM began using their own reed valves and there have been complaints about these breaking or not sealing properly. Next are all the KTM known issues not serious enough to issue a product recall. Each year KTM dealers receive a list of these which we can't access, but you'll occasionally see the issues pop on forums like this one: 2016 KTM 125 SX and 150 SX models have been found to have cracks in engine cases. A minor but persistent KTM problem, the weak sidestand on the Husqvarna enduro models too. In the quest for light weight most European bikes have weak sidestands but KTM are probably the worst. If it falls off just buy a decent aftermarket one. This isn't a recall problem of course. KTM's TPI issues and oil injection problems for Husqvarna coming soon!So it comes down to scouring the forums for KTM problems, recalls, and known issues, chatting with owners, and trying to sort out the truth from the alarming amount of bullshit on the internet. Until the new design in 2017, the two stroke starter motors were widely seen as a very unreliable weak design that needed frequent maintenance. And this brings us up to the new Husqvarna TPI and KTM oil injection problems. If you have an early TPI model keep an eye on this. What about KTM frames breaking? KTM parts were ridiculously expensive. Neither are Sherco parts, I think both could learn a lot from Gas Gas, Beta, and of course the Japanese, on their parts and bike pricing. I remember when I posted a comprehensive vid about known issues with Beta a pile of KTM riders laughed and said that's why they didn't buy Italian shit. Well I think this video shows it's just a matter of perception unless you are one of the sheep who are blindly into brand loyalty. Guys love posting these pics online and saying KTM have gone too far with trying to reduce weight. In 2017 plenty of Husqvarna riders reported their bikes were running perfectly but a substantial number reported issues with starting, idling and running lean. Hopefully there have been mapping upgrades for KTM 2018 models. A fairly rare KTM & Husqvarna issue was the weak idler gear on 2017 models, but it would be a much more common problem if riders used their kick starters a lot. Apart from product recalls on certain 4CS forks on Husqvarna and KTMs, they were very widely criticised for being much worse than the earlier forks. Associated Milk Producers is voluntarily recalling the FDA-regulated product, whey powder. Now the U.S. Department of Agriculture’s Food Safety and Inspection Service has issued an alert. They are alerting companies and the public of any food that may contain this ingredient. Some of those are snacks such as Ritz crackers and Swiss Rolls.
